We are seeking qualified candidates for the Full-Time Rehabilitation Counselor position. The position is responsible for providing care and supervision to all assigned clients, ensuring a safe, healthy, secure, and pleasant environment that promotes growth and development.
Other Responsibilities, but not all-inclusive:
- To facilitate client growth and development toward independence in self-care, daily living skills, socialization, and community living.
- To participate in the development of the individual service plans and related documentation.
- Provide guidance and support to all clients; teach and reinforce skill development in the areas of nutrition, basic self-care, routine medical care, and follow-up, as developed through the joint planning sessions of the individual service plans.
- Review and modify individual service plans to accommodate growth and development.
- Successful completion of medication training. Maintain and monitor medications consistent with individuals’ self-medication plans, in compliance with State and Agency medication administration regulations.
- Track all client progress, as identified in their individual service plans. Provide supplemental reports on individual growth and development, as required. Record their progress as they move through the service delivery system.
Qualifications:
- High school diploma or equivalent required; Associate’s degree preferred in the social sciences or health care-related field and one year of relevant experience in the MH/MR field; or any combination of experience, education, or training that would provide the level of knowledge, skill, and ability required.
- Good communication skills, both oral and written.
- Ability to work independently and as part of a team.
Scope:
- Requires the ability to lift, carry, fold, kneel, reach, stack, stoop, and bend, which may include direct intervention with residents.
- Possess manual dexterity and fine motor skills.
- May be required to possess a valid driver's license and an acceptable driving record (contingent upon facility location and client and program needs).
- May require crisis intervention in emergencies.
- May be required to report to work during emergencies, including inclement weather.
