Rehabilitation Nurse Practitioner

Position Overview

This position offers a unique opportunity to work across multiple rehabilitation environments while developing strong long-term relationships with patients recovering from injury, illness, surgery, and chronic pain conditions. The schedule is flexible and designed to support work-life balance.

Practice Settings & Responsibilities

Acute Rehabilitation Hospital

• Perform follow-up patient visits

• Assist with discharge planning and coordination of care

• Collaborate closely with rehabilitation teams and specialists

Skilled Nursing Facility

• Provide rehabilitation-focused medical management

• Conduct pain management consultations

• Monitor patient progress and recovery plans

Outpatient Office

• See follow-up patients discharged from acute rehab or skilled nursing facilities

• Provide ongoing rehabilitation and pain management care

• Coordinate continuity of care across treatment settings

Qualifications

• Licensed Nurse Practitioner

• Strong clinical and communication skills

• Ability to work independently and collaboratively

• Experience in rehabilitation medicine preferred, but not required

• New graduates with strong interest in rehabilitation medicine are encouraged to apply

Compensation & Benefits

• Salary: $125,000 – $140,000 annually based on experience and qualifications, plus bonus

We are seeking a full-time Nurse Practitioner to join our expanding team and provide care in acute rehabilitation, skilled nursing facilities, and outpatient settings.
