Overview
We’re hiring a Clinical Pharmacist to support Medication Therapy Management (MTM) services, including Comprehensive Medication Reviews (CMRs) and patient consultations focused on statin use per AHA guidelines. This role is heavily phone-based and requires strong clinical judgment, documentation accuracy, and comfort working in web-based systems.
Job Responsibilities
What You’ll Do
Conduct outbound calls (target volume: ~100 calls/day) to complete CMRs and MTM services
Provide patient consultation and education, including statin-related guidance aligned with AHA guidelines
Accurately document/transcribe patient-specific:
prescription details
diagnoses
recommendations
into a third-party web-based application
Contribute to standardized documentation that will be mailed to patients
Attend trainings to build/refresh role-specific skills

Qualifications
Required Qualifications
Active, valid Pharmacist license in Florida (FL)
Please include a screenshot of your FL license at the top of your resume
4+ years experience as a clinical MTM pharmacist (management experience preferred)
Experience with Medicare Part D Star measures, especially SUPD, SPC, and CMR
Strong knowledge of medical/pharmacy terminology and medication review best practices
Strong English communication skills (verbal + written)
Tech-savvy; comfortable moving quickly in online tools/applications
Ability to maintain confidentiality; detail-oriented; organized; sense of urgency
Preferred
Salesforce experience
