Key Responsibilities:

- Monitor and assess patient health data collected through remote monitoring devices

- Communicate with patients and their healthcare team to provide updates on patient progress and address any concerns or issues

- Collaborate with other healthcare professionals to develop and implement individualized care plans for patients

- Educate patients and their families on how to use remote monitoring devices and manage their health conditions

- Maintain accurate and up-to-date patient records and documentation

- Stay current on industry developments and best practices related to remote patient monitoring

- Participate in regular team meetings and training sessions to enhance skills and knowledge

Qualifications:

- Active nursing license in the state of Idaho

- Minimum of 2 years of experience in a clinical setting, preferably in home health or remote patient monitoring

- Strong understanding of chronic disease management and remote patient monitoring technology

- Excellent communication and interpersonal skills

- Ability to work independently and remotely

- Comfortable with technology and able to quickly learn new systems and software

- Bilingual (English/Spanish) skills are a plus
