Job Overview:

The Fountains at Hosanna! is conveniently located in Lakeville, adjacent to Hosanna Church. We are less than a mile from Highway I-35 and near County 77. The Fountains offers seniors 92 Independent Living, Assisted Living, and Memory Care apartments. We are known for our exceptional staff, who create a warm, friendly, fun community.

 

We are looking for a Resident Assistant/Home Health Aide who would provide personal cares to improve and/or maintain the senior assisted living residents’ physical and emotional well-being for our senior living in Lakeville, MN.

 

This caregiver schedule includes;

• 56 hours every two weeks ; Full Time
• Evening Shift
• Block Schedule:
  • Week 1: Monday (2p-10p) , Wednesday (4p-10p) , and Friday (2p-10p)
  • Week 2: Monday (2p-10p) , Wednesday (4p-9p) , Friday (2p-10p) , Saturday (2p-10p) , and Sunday (2p-10p)
• Shift differential pay available
• Every other weekend and holiday rotation

Responsibilities

• Personal Care Assistance:
  Supports residents with daily living activities, including:

  • Bathing
  • Foot and fingernail care
  • Shaving
  • Dressing
  • Hair care
  • Feeding
  • Toileting
  • Assistance with hearing aids and/or glasses
  • 1-2 person transfers and use of mechanical lifts

• Health-Related Support:

  • Assists with treatments as outlined in individualized care plans under the supervision of a Registered Nurse (e.g., blood glucose monitoring).
  • Administers or assists with medications, including insulin injections, as directed by a Registered Nurse.
  • Takes and records vital signs such as temperature, blood pressure, pulse, and respiration.

• Daily Living Support:

  • May assist with household tasks including laundry, cleaning, meal service, and resident activities.

• Resident Observation & Reporting:

  • Monitors and reports changes in residents’ physical or emotional condition (e.g., mood, appetite, weight).
  • Communicates observations to appropriate staff and contributes relevant input to care planning.
  • Acts as a mandated reporter, promptly reporting any concerns related to potential mistreatment.

• Emergency Response:

  • Responds appropriately to resident needs and building emergencies, following established protocols.

Required Qualifications

• less than 1 year of experience
• Ability to use technology in the form of computers, handheld devices, cell phones, and fax machines
• If under 18, must be on the MN NA registry

Preferred Qualifications

• 1-3 years of experience

Qualifications:

Compensation: $21.75 - $30.31 Hourly
