Description

Ivybrook Assisted Living is looking for a dedicated Resident Care Coordinator. The Resident Care Coordinator (RCC) serves as a liaison with hospital personnel, physicians, community organizations and other health related service agencies to provide quality care to our assisted living residents. Additionally, this role is responsible for hiring, training, and managing a staff that is capable of providing superior care, while maintaining the physical and emotional health of residents, and in accordance with current federal, state, and local standards, guidelines, and regulations, while demonstrating Ivybrook's principles and core values.Essential Job Duties:Conduct and coordinate the potential assisted living resident assessment program in accordance with current rules, regulations, and community policies and procedures that govern resident assessment, including ensuring that an initial assisted living resident assessment is completed prior to each resident's move-in date and a comprehensive care plan is completed upon move-inConduct routinely scheduled re-assessments on each assisted living resident, and change of condition assessments as neededEnsure each assisted living resident is receiving the best possible care and programs to meet their individual needsConduct interviews and observations of the assisted living residents on a continual basisAlert the Executive Director, Director of Nursing, family members and the others (As necessary) of any changes in the physical or emotional health of the resident, and the department's ability to meet those needsRecruit, interview, hire, manage, schedule, motivate, evaluate, and supervise department staff in accordance with community policies, procedures, and established budgetDaily review of timekeeping, meal breaks and overtimeCreate and conduct orientation for new associatesEnsure ongoing training and education for all associates, in accordance with all governing requirements, and community policies and procedures to ensure that staff is properly trained in competent delivery of medications to residents, safety and emergency procedures, documentation/record keeping, resident's record and acceptable treatments, etc., and in providing the best care possible for our assisted living residents and ensure that all care staff completes state required trainingPerform all administrative duties accurately and timely, including associate evaluations, necessary forms, reports, evaluations, studies, surveys, resident records, assessments, care profiles, etc., including:Coordinate departmental schedule to ensure adequate staffing in accordance with legal requirements, community standards, policies, procedures, and budgetsEnsure coverage of job duties within the department during associate absences, either through delegation or personal completion of dutiesMaintain inventory of supplies necessary for resident careEnsure continuity of the assisted living resident's total care regimen by developing methods for the coordination of nursing services with other resident servicesWork closely with all departments in the community to ensure that residents are receiving all available services to enhance their quality of lifeAct as a resource and care manager for the residents and their familiesContact physicians, family, and others to obtain orders for care and represent the community Coordinate prescription orders with doctors' offices, as well as pharmacy delivery of medicationOversee the central storage, tracking and delivery of medicationsEnsure all medication carts are in orderEnsure all medication records administration are completed per company guidelines and state regulationsResponsible for the overall safety and health of all assisted living residentsEnsure that all governing requirements are closely monitored and executedEnsure that all physician orders are carried out properlyAid in the community marketing effort through positive interactions, acting as a liaison between the community, and families/outside health service providersKey management member acting as Manager on Duty when necessaryNon-Essential Job Duties:Perform other job duties or special projects assigned/requested by the Executive DirectorWork with Ivybrook and outside resources to develop and continue a productive support group to be offered as a resource to the family members of our residentsUnderstand and comply with all federal, state and local regulations, and all company policies and procedures concerning the departmentDisplay tact and friendliness when dealing with residents, associates and guests Promote and lead with a spirit of teamwork and open communication in accordance with the Ivybrook principles and core valuesAssist the Executive Director in completing an annual budget, including ensuring that the financial goals are met monthly and manage labor and other expenses to meet these financial goalsPossesses the ability and desire to minimize waste and misuse of supplies/equipmentMaintain a safe and secure environment for all staff, residents, and guests following established safety standards policies, and proceduresSupervisory/Management Responsibilities (Job Title(s) & number of employees):Caregiver = 8-12 employeesMedication Technician =2-12 employeesMinimum Job Requirements (Include education, experience, special skills, licenses, certifications):High school diploma or GED equivalent is requiredPrior related work experience functioning in a similar assisted living management role in the industry or similar work environment is essentialPrior management or supervisory experience is requiredCurrent CPR/First Aid Certification is requiredMust complete a health screening and provide negative TB test results (must be within the last 6 months or 14 days of employment)Must have solid pc skills and be familiar with several Microsoft Office Suite software programs (e.g. Word, Excel, Outlook, Email, etc.,) and other office equipment (e.g. scanners, copiers, and fax machines)Excellent communication skills are required, including the ability to speak, read, and write English. Must have basic math skills (addition, subtraction, and multiplication) as required for personnel actions, budget and other financial responsibilities.Must possess the ability to make independent decisions when circumstances warrant such action, and to remain calm during emergency situationsMust possess the ability to deal tactfully and professionally at all times with personnel, residents, family members, and guests.Preferred Job requirements (include lifting, carrying, pushing, pulling, bending, walking, standing, sitting):Must be able to move intermittently throughout the workday and throughout the communityAbility to lift/carry, push, and pull up to 40+ poundsAbility to assist in the moving of residents when necessary
