Essential Responsibilities

Provide high-quality nursing care and treatment in accordance with each resident's individualized service plan.

Implement, evaluate, and coordinate nursing services to ensure exceptional resident outcomes.

Observe, assess, and document resident signs, symptoms, behaviors, and responses to treatment.

Support compliance with infection prevention protocols, universal precautions, and all applicable state and federal regulations.

Coordinate the training and daily work activities of Resident Assistants based on competency and skill level.

Assist with staffing coverage and scheduling needs to ensure quality resident care.

Provide feedback regarding staff performance and recommend coaching or corrective actions when appropriate.

Foster a positive clinical environment that promotes employee engagement, growth, learning, and job satisfaction.

Identify resident and staff concerns and collaborate with the interdisciplinary team to develop effective solutions.

Accurately implement physician orders and coordinate ongoing medical care.

Order medications from the pharmacy and ensure proper distribution to medication carts.

Maintain accurate and timely documentation, reporting, and recordkeeping.

Communicate effectively with physicians, home health agencies, residents, and family members regarding health conditions and care needs.

Coordinate laboratory testing and follow up with physician offices regarding results and treatment recommendations.

Assist with resident admissions, move-ins, transfers, and discharges.

Provide comprehensive shift reports regarding resident status, changes in condition, and care needs.

Maintain confidentiality and demonstrate professionalism in all interactions.

Understand and support Resthaven policies, procedures, and mission.

Participate in the clinical on-call rotation.

Supervise, coach, mentor, and support nursing staff to promote quality care and team development.

Perform additional duties and responsibilities as assigned.

Education, Training, & Experience

Current LPN (Licensed Practical Nurse) or RN (Registered Nurse) license preferred, but not required.

Previous nursing experience preferred, particularly in assisted living, senior living, long-term care, memory care, or dementia care settings.

Demonstrated clinical competency and knowledge of nursing best practices.

Experience leading, coaching, or supervising healthcare staff preferred.

Proficiency with Microsoft Office, including Word, Excel, and email systems.

Strong written and verbal communication skills.

Ability to communicate effectively with residents, families, physicians, and team members while recognizing age, cultural backgrounds, cognitive abilities, and physical conditions.

Knowledge and application of dementia-specific interventions preferred.

Strong organizational skills with attention to detail and the ability to prioritize multiple responsibilities.

Ability to work independently and effectively manage time.

Ability to remain calm, resourceful, and responsive during urgent or emergent situations.

Ability to operate standard office equipment including telephone, copier, fax machine, and calculator.

Participation in all required in-service education, mandatory training, and departmental meetings.

Current nursing license, certifications, and continuing education maintained in accordance with state requirements.

Physical Requirements

This role is active and hands-on. You must be able to:

Ability to frequently stand, walk, bend, stoop, kneel, and reach throughout the workday.

Ability to lift, transfer, push, and pull up to 50 pounds independently and heavier weights with assistance.

Ability to assist residents with mobility, transfers, and activities of daily living as needed.

Ability to respond quickly and appropriately during emergencies.

Manual dexterity sufficient to administer medications, perform assessments, and document care accurately.

Visual and auditory acuity necessary to observe residents, read medical records, and communicate effectively.

Ability to work in a fast-paced healthcare environment with frequent interruptions and changing priorities.

Ability to tolerate exposure to infectious diseases, cleaning chemicals, bodily fluids, and other environmental conditions common in healthcare settings.

Ability to work flexible schedules, including weekends, holidays, evenings, and participation in the clinical on-call rotation as needed.
