    Location: Bronx, NY 10463 (Riverdale area)
    Employment Type: Full-Time | Permanent
    Schedule: Sunday – Thursday, 9:00 AM – 5:00 PM (rotating holidays)
    Compensation: $67,000 – $72,000 per year + Full Benefits

    A respected, mission-driven senior care organization in the Bronx is seeking a compassionate and experienced Resident Services Coordinator to join its team. This organization is dedicated to enhancing the quality of life for seniors through comprehensive care, support services, and a collaborative, resident-focused approach.

    Position Summary
    The Resident Services Coordinator plays a key role in supporting the Social Services Department and ensuring residents receive high-quality, individualized care. This position focuses on care coordination, resident advocacy, and supporting families through the complexities of long-term care and aging services.

    Key Responsibilities

    • Assist in the overall management and daily operations of the Social Services Department

    • Support and guide a multidisciplinary care team to deliver resident-centered services

    • Provide emotional, social, and community-based support to residents and their families

    • Conduct resident assessments and coordinate case management services

    • Assist residents and families in navigating long-term care resources, benefits, and entitlements

    • Maintain accurate, timely, and compliant documentation of services and interactions

    • Promote and uphold quality of life standards in alignment with regulatory and organizational guidelines

    Qualifications

    • Master’s degree in Social Work (MSW) required

    • NY State LMSW (or working toward licensure) preferred

    • Minimum of 2 years of experience in geriatrics, long-term care, or case management preferred

    • Experience with discharge planning and assessing the needs of an aging population preferred

    • Background in healthcare, aging services, or long-term care settings a plus

    • Strong communication, organizational, and documentation skills

    • Ability to collaborate effectively with interdisciplinary teams
