Job Title: Residential Care Provider (RCP)

Job Code: SSS010/ Client Support Worker or T6001/Support Services Worker

Shift: Full-Time (40 hours per week onaverage)/PRN 19 hours per week on average)

Job Hours: Times May Vary

Base Location: Lawrenceville, GA.

Division/Department/Program: Campbell Gate IDD ResidentialHome

The Residential Care Provider monitors general resident activity to observe and report behavior/progress of individuals with severe developmental disabilities living in a group home or other community residential setting and provides individualized skills trainings according to residents' needs. Monitors general resident activity to observe and report behavior/progress of the individual(s) with severe developmental disabilities or mental illness living in a group home or other community residential setting. Interacts with the individual positively and respectfully. Uses age and gender appropriate language. Trains individuals in appropriate social interactions to promote positive images and culturally typical behaviors. Determines the individuals' strengths, needs, abilities, and preferences by observing daily activities and reviewing records. Assists individual in basic problem solving and daily decisions making using least intrusive methods appropriate for each. Acknowledges and praises their choices.

Duties & Responsibilities:
  • Assist individuals with Developmental Disabilities in basic daily living skills.
  • Carry out a person-centered Individual Service Plan (ISP)
  • Ensures individuals are dressed neatly and appropriately and are groomed at all times
  • Assists and encourages individuals to care for immediate personal needs such as toileting, washing their hands, and eating
  • Teaches individuals daily skills such as household chores, laundry, cleaning, and washing dishes, as appropriate according to their individual plan
  • Transports individuals to work, day programs, doctor appointments, shopping, recreational activities
  • Assist with scheduling medical appointments
  • Assists in the implementation of individual and behavioral plans and provides feedback for plan goals
  • Assists individuals in the pursuit of recreational, social and community activities based on their preferences and individualized treatment plans
  • Assists and encourages individuals in the establishment of social networks
  • Promotes participation and involvement in community outings
  • Ensures each individual's rights are protected
  • Treats individuals with dignity and respect
  • Maintains the residence in a clean and organized condition
  • Other duties as assigned
Minimum Qualifications:

High school diploma or GED and six (6) months of relevant job-related training or experience.

Please Note:
  • Must be 21 years of age.
  • Must have valid Georgia Driver's License, and a clean motor vehicle report.
Preferred Qualifications:
  • Experience working with MH/IDD population.
  • Certified Nursing Assistant certification (CNA), Direct Support Professional (DSP), Direct Care Professional (DCP) or Patient Care Technician (PCT) preferred.
Requirements/Competencies:
  • Must pass a pre-employment drug screen and subject to random drug screens.
  • Must have a valid Driver's License and a clean Motor Vehicle Record in accordance with company policy.
  • Must pass a criminal background check (including fingerprinting)
  • Requires TB Evaluation at pre-employment and annually.
  • Must be able to lift 20 pounds.
  • Requires long periods of sitting/standing.
  • Must have the following technical skills - Word, Excel, and Electronic Health Record (Care Logic)
  • Must be willing to transport residents in the company van.
  • Must demonstrate the ability to maintain professional boundaries with clients in a residential treatment setting.
  • Must be able to lift and carry groceries and supplies and move furniture for cleaning.
  • Must be able to assist disabled individuals on/off vehicle and secure seatbelts.
  • Must be able to use a gait belt and mechanical lift to lift and position individuals, as needed.
Note: Some positions may require a valid driver's license.
