RESIDENTIAL HABILITATION COORDINATOR

Well-established non-profit supporting adults with disabilities is seeking a Residential Habilitation Coordinator. Hours are Monday - Friday 9:00am - 5:00pm. Company offers medical, dental, vision, life, disability, flex spending, tuition assistance, 403B, retirement, and PTO.

Responsibilities:
-Development, implementation, and ongoing evaluation of individualized habilitation plans for people with intellectual and developmental disabilities residing in client organization residences.
-Ensure that all services are delivered in accordance with Person-Centered Planning principles, regulatory requirements, and best practices.
-Provide oversight of Staff Action Plans and be accountable for ensuring the Direct Support Professionals (DSPs) are effectively trained and support consistent implementation of plans as written.
-Monitor individual progress toward outcomes, ensure health and safety needs are met, and drive continuous improvement in service delivery through data-informed decision-making.
-Supervise Residential Habilitation Specialists.
-Working collaboratively with the Residential Director and interdisciplinary team members, play a key role in aligning daily support with each individual s Life Plan and ensuring compliance with all applicable regulatory and agency standards.
-Report to the Director of Residential Services.
