    What We're Looking For
    We are seeking a compassionate and organized Restorative Nurse to oversee and coordinate restorative nursing programs for our residents. This role works closely with therapy, nursing, and interdisciplinary teams to maximize resident independence and maintain the highest level of functioning.

    Key Responsibilities

    • Develop, implement, monitor, and evaluate individualized restorative nursing programs.
    • Collaborate with therapy and nursing staff to identify residents who would benefit from restorative services.
    • Train and supervise restorative nursing assistants and ensure program compliance.
    • Track resident progress, outcomes, and participation in restorative programs.
    • Complete restorative nursing documentation and ensure accuracy of records.
    • Participate in care plan meetings and interdisciplinary team discussions.
    • Assist with the MDS process, including gathering restorative nursing data, documenting restorative services, supporting MDS assessments, and helping ensure accurate reimbursement and regulatory compliance.
    • Ensure compliance with state, federal, and facility guidelines related to restorative nursing services.

    Qualifications

    • Active Illinois RN or LPN license.
    • Experience in long-term care or skilled nursing preferred.
    • Knowledge of restorative nursing programs and MDS documentation.
    • Strong communication, organizational, and leadership skills.
