Speech Language Pathologist Assistant
Circle of Care Pediatric Home Healthcare Company is looking for individuals to fill immediate positions to provide Speech Language Therapy to patients ranging in age from birth through age 20. These services are provided in the home, daycare or community setting as prescribed by the attending physician.
This opportunity will begin as PRN and then build to full-time with benefits.
Job Summary:
The speech pathologist/audiologist assistant is a qualified professional person who provides speech therapy services to patients in the home as prescribed by the attending physician.

Some of the Responsibilities include:
1. Plans activities and provides therapeutic services for speech and language disorders
2. Records type of treatment and patient's reaction to it on clinical/progress notes, which are written on the day of the visit and incorporated in the patient's record weekly.
3. Maintains adequate records on all patient's, including summary reports
4. Utilizes community resources and Agency personnel for proper referral
5. Be supervised by an SLP as follows: Minimum of two hours per week (1 hour direct, on site and 1 hour indirect). Follow guidelines for supervision as outlined by the State Board for Speech Language Pathology & Audiology
6. Attends case conferences as requested
7. Confirms, on a weekly basis, the scheduling of visits with the Patient Care Coordinator and/or ST Supervisor to coordinate necessary visits with other personnel
8. Participates with staff, supervising SLP, patient, and physician in discharge planning activities and completes the speech therapy discharge summary within the designated framework of the Agency policies.
