Responsibilities

The RN Admissions is responsible for receiving admission, discharge and transfer information to facilitate patient placement in appropriate bed assignments. The ability to assess chart reviews, case management and patient flow, while working collaboratively with providers, is essential. Responsible for various aspects of acute care coordination through intake, admission and transition. In collaboration with the Command Center, the RN Admissions will assist in identifying patients appropriate for inpatient or observation admission status. This position oversees, coordinates and communicates discharge planning services for patients through the healthcare continuum. Collaborates with the multidisciplinary team on discharge processes and serves as a liaison on community facilities and resources.

Qualifications

Education & Experience

  • Associate''s Degree in Nursing required
  • 2 years of experience in an acute care setting required
  • 3 years of experience in Registered Nursing across a wide spectrum of Inpatient and ED specialties preferred

Licenses and Certifications

  • RN - Current and valid Registered Nurse license to work within the state of California required
  • BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
  • ACLS - Advanced Cardiac Life Support Certification required
