As an Admissions Nurse for our Hospice team, you will:
Traveling to CHLs and homes to see our hospice clients and their families
Complete initial, resumption, follow up, a significant change of status and discharge assessments including comprehensive head to toe assessment, HIS assessment, and all associated assessments with admission or discharge
Initiate an individualized plan of care, and update the plan of care for resumptions, significant change in status, and re-certifications.
Use health assessment data to determine nursing diagnosis
Obtain, implement, perform and maintain orders as received by physicians overseeing patient care services
Initiate appropriate preventive and rehabilitative nursing procedures. Administer medications and treatments as prescribed by the physician
Counsel the patient and family in meeting nursing and related needs
Provide health care instructions to the patient as appropriate per assessment and plan of care
Assist the patient with activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care.
