As an Admissions Nurse for our Hospice team, you will:
- Traveling to CHLs and homes to see our hospice clients and their families
- Complete initial, resumption, follow up, a significant change of status and discharge assessments including comprehensive head to toe assessment, HIS assessment, and all associated assessments with admission or discharge
- Initiate an individualized plan of care, and update the plan of care for resumptions, significant change in status, and re-certifications.
- Use health assessment data to determine nursing diagnosis
- Obtain, implement, perform and maintain orders as received by physicians overseeing patient care services
- Initiate appropriate preventive and rehabilitative nursing procedures. Administer medications and treatments as prescribed by the physician
- Counsel the patient and family in meeting nursing and related needs
- Provide health care instructions to the patient as appropriate per assessment and plan of care
- Assist the patient with activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care.
