Hospice of Marion County, a part of Empath Health, is seeking a caring and motivated Admissions RN for admissions in homes, facilities, and hospitals in Marion County.

Schedule: PRN, minimum of 64 hours a month availability.

What Youll Do

• The Admissions RN is a first responder who assesses eligibility for Empath Health Network of Care. • Advocating for optimal access to the right care at the right time in any care setting. • Completes the admission process based on care program specifics, guidelines, and patient-specific needs. • Coordinates and assists with the Professional Liaison to deliver educational, in-service, and outreach tools in the hospital. • Works directly with community referral sources to educate regarding Empath Health programs and services. • Complies with RN On Call scheduling needs as well as vendor credentialing process.

Requirements

Education and/or Experience: Associate's degree (A.A.) or equivalent from two-year college or technical school; or six months to one year related experience and/or training; or equivalent combination of education and experience.

Licensure: Valid Florida license as a Registered Nurse.

Valid Florida State Driver's License.

For field-based positions, employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance, a copy of which will be placed in the employee's HR file.
