Job Summary
The primary role of the RN MDS Assessment Coordinator is to assist in gathering of clinical documentation and completion of the non-comprehensive MDS assessments and ensures that essential information gathered during the assessment period, is integrated in the care planning process.
Delegation of Authority
As the RN MDS Assessment Coordinator, you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.

Duties and Responsibilities
Administrative Functions
- Complete and oversee MDS assessments as directed by Director of MDS
- Ensure that the facility adheres to all regulatory requirements and current standards of practice in relation to the MDS care planning process
- Educate the interdisciplinary team about accuracy of clinical documentation and its importance in all aspects of care delivery
- Coordinate the timely completion of the MDS assessments with the interdisciplinary team within the guidelines and specified time frames as per RAI Manual
- Assist in the review of the clinical record accuracy of MDS coding and completeness of documentation.
- Coordinate Case Management process
- Act as liaison to the interdisciplinary team in the discussion of essential findings of a resident’s condition obtained while completing MDS assessments
- Assist in transmission of MDS assessments, and obtains validation records and reviews them for needed corrections
- Other duties as assigned or as deem necessary and appropriate, as indicated by Director of MDS
Education
Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved RN program
Experience
- Experience in long-term care and one-year job-related experience in the MDS process
- Must have familiarity with various computer software
- Must be efficient, organized, detailed –oriented and flexible
- Must have excellent time management and consistently meet task deadlines

- Must possess the ability to make independent decisions when circumstances warrant such action
- Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public
- Must possess the ability and willingness to work harmoniously with other personnel
- Must have patience, tact, a cheerful disposition and enthusiasm
- Must meet physical and sensory requirements (with or without the aid of mechanical devices)
- Must possess good communication skills and be able to relate professionally and positively to residents, resident’s family members, and facility staff
- Must meet all local health regulations, pass post-offer drug test if required, and pass post-employment physical exam if required
- Must be capable of performing the responsibilities of this job, with or without reasonable accommodation.
