    The Work You’ll Do

    As our RNAC, you’ll use your clinical knowledge and attention to detail to connect assessment, documentation, and care. You’ll:

    • Coordinate accurate and timely MDS assessments and submissions.
    • Assess new admissions and changes in resident condition and level of care.
    • Coordinate the interdisciplinary MDS and care-planning process.
    • Lead and participate in resident care conferences.
    • Develop, review, and revise individualized plans of care with the interdisciplinary team.
    • Monitor resident outcomes and ensure care plans evolve as residents’ needs change.
    • Maintain required documentation and support state and federal regulatory compliance.
    • Support quality improvement, surveys, and other essential clinical needs of the facility.

    You understand that behind every assessment, MDS, and care plan is a person. You bring the clinical judgment to recognize what residents need, the attention to detail to make sure it is captured accurately, and the commitment to help turn that information into good care.

    You’ll also bring:

    • A resident-first approach and a commitment to quality care.
    • Strong clinical judgment, organization, and attention to detail.
    • The ability to collaborate across nursing and the interdisciplinary care team.
    • A current and active Registered Nurse (RN) license.
    • Experience with MDS, resident assessment, care planning, or long-term care.
    • Knowledge of applicable state and federal long-term care requirements.
