TAG MedStaffing is hiring an RN Assessment Manager (Temporary) in Bronx, NY for our client, supporting clinical assessments, UAS-NY evaluations, care planning, and enrollment activities for older adults receiving long-term care services. This field-based opportunity will conduct in-home assessments, determine program eligibility, and collaborate with interdisciplinary teams to ensure accurate clinical documentation and continuity of care. The ideal candidate will bring strong UAS-NY experience, managed care knowledge, and the ability to work independently across multiple service areas.

Responsibilities as the RN Assessment Manager:

• Clinical Assessments: Conduct in-home and virtual assessments utilizing UAS-NY to evaluate eligibility, care needs, and service requirements for prospective and enrolled participants.
• Care Planning: Develop service recommendations, complete comprehensive assessments, and support care planning activities based on clinical findings.
• Documentation Management: Maintain accurate and timely clinical documentation, assessment records, service plans, and electronic medical records within established deadlines.
• Clinical Review: Evaluate diagnoses, medications, prior assessments, and changes in condition to ensure accurate clinical records and appropriate care coordination.
• Interdisciplinary Collaboration: Partner with enrollment, care management, quality, and clinical teams to facilitate participant onboarding, reassessments, and transitions of care.
• Quality & Compliance: Ensure compliance with UAS-NY requirements, managed care regulations, and organizational policies while supporting quality improvement initiatives.

Qualifications for the RN Assessment Manager:

• Education: Bachelor’s degree in Nursing or a related healthcare field is preferred.
• Licensure: Active and unrestricted New York State Registered Nurse (RN) license is required.
• Experience: 5+ years of experience in care management, case management, utilization review, discharge planning, population health, or related clinical functions is required.
• Industry Knowledge: Experience within managed care, care management, home care, community-based healthcare, or population health programs is required. Experience supporting adult or senior populations is preferred.
• Technical Skills: Proficiency with clinical assessment platforms, UAS-NY, electronic medical record systems, and Microsoft Office applications is required.
• Skills & Attributes: Strong assessment, clinical judgment, communication, organizational, and customer service skills with the ability to work independently, manage multiple priorities, and travel throughout assigned service areas are required.
