Description:
• A leading nonprofit supporting individuals and communities through mental health, social services, and recovery programs is seeking a full time Registered Nurse (RN) for their Behavioral Health CTI team.

POSITION SUMMARY:
• Join an innovative, Governor's Office–backed program designed to support vulnerable New Yorkers during critical life transitions.
• As the CTI RN, you'll provide community-based medical and health support, working with a multidisciplinary team to bridge gaps in care and connect participants to essential services.
Key Responsibilities:
• Assess and monitor participants' physical, cognitive, and psychosocial health.
• Support transitions from hospital/ED to community with care planning and follow-up.
• Provide health education, medication reconciliation, and disease prevention guidance.
• Link participants to community resources and healthcare providers.
• Collaborate with clinicians, care managers, and peer specialists to ensure continuity of care.
• Conduct field visits and provide flexible, community-based support.
Qualifications:
• NYS RN license and registration required.
• Minimum 1 year of post-licensure nursing experience preferred, ideally with behavioral health populations or community/home visits background.
• Strong knowledge of health promotion, disease prevention, and community resources.
• Ability to work flexibly in field-based settings.
