This position is responsible for coordinating peer review, performance improvement, and patient satisfaction activities. Additionally, the position is responsible for developing new and revising existing forms as needed to comply with regulations and standards or to meet needs identified by the team. Also, the position will provide services to the patients and a linkage/referral source to include applicable pharmacy assistance programs.
Role & Responsibilities:
- Demonstrates knowledge and skills necessary to provide ongoing written support for all aspects of medical records; thoroughly documents Case Management activities.
- Collects vital signs and enters data into the NextGen HER as needed.
- Screens medical requests/patient appointments.
- Assists patients by linking and referring appropriately to services not provided by BHS per provider instructions.
- Maintains the medication room following BHS policy and procedures.
- Cross covers group therapy activities as needed.
- Assists patients in applying to pharmaceutical companies' patient assistant programs as appropriate by providers.
- Acts as liaison to the pharmaceutical company representatives requesting samples and other materials as needed.
- Logs in samples and PAP medications into the appropriate log per procedure.
- Periodically checks expiration dates on samples and disposes of expired samples per procedure.
- Coordinates lunch presentations weekly with pharmaceutical representatives.
- Always maintains patient confidentiality and privacy according to SCH policies and procedures; uses discretion when discussing patient matters.
- Demonstrates courteous and cooperative behavior.
- Maintains positive working relationships.
- Exhibits professional standards through appearance and demeanor consistent with SCH standards.
- Demonstrates thorough knowledge of work assignments and performs assignments in a manner to meet quality and timeliness objectives.
- Demonstrates knowledge and understanding of SCH policies and procedures.
- Complies with applicable safety policies and procedures.
Essential Skills:
- BILINGUAL English-to-Spanish and Spanish-to-English A PLUS!
- Must have computer experience in Power Point, Microsoft Excel, and Microsoft Word.
- Must compile reports, data, and minutes for Peer Review, Performance Improvement, and Patient Satisfaction.
- Must have excellent skills in patient communication and education.
- Case Management experience preferred.
Qualifications & Education Requirements (add Licensure on pertinent jobs):
- Must be a graduate of an accredited College of Nursing School.
- Must have a current nursing license in each state of desired practice.
- Annual basic BLS/CPR certification is also required.
- Previous experience working as an RN is preferred.
