Position: RN – Bridge to Home
Location: Brooklyn, NY 11206
Duration: 10/26/2026 – 01/23/2027
Shift: 8:30 AM–8:00 PM / 8:00 PM–8:30 AM
Schedule: 34.5 Hours/Week | 3 Days/Week | 11.5 Hours/Day | 2 Day Shifts + 2 Night Shifts | No On-Call
Pay Rate: $70-$75/hr..
Requirements
Required Experience
- Experience caring for patients with serious mental illness (SMI) is strongly preferred.
- Experience in healthcare settings caring for individuals experiencing homelessness, mental illness, substance use disorders, or justice involvement is strongly preferred.
- Candidates without substantial prior experience should demonstrate a strong desire to learn and care for this population.
Required Education
- For nurses issued a NYS RN license on or after July 1, 2020, a Bachelor of Science in Nursing (BSN) or higher, or other nursing degree identified by NYSED, is required.
Required Licensure & Certifications
- Valid and current New York State Registered Professional Nurse License.
- Current AHA BLS certification.

Responsibilities
- Assess patients through interviews, chart reviews, and referrals.
- Educate and teach patients based on diagnosis and individual needs.
- Administer medications and initiate appropriate nursing interventions.
- Educate patients and families regarding treatment, medications, diagnostic procedures, and follow-up appointments.
- Assist patients with maintaining an accurate medication list and understanding prescriptions.
- Coordinate responses to medical and psychiatric emergencies.
- Schedule nurse follow-up visits and assist patients in accessing specialty services.
- Triage patients and perform vital sign checks.
- Document observations, evaluations, and interventions in electronic medical records.
- Participate in preventive health measures, including vaccine administration.
- Administer nebulizer and/or metered-dose inhalation therapy.
- Apply harm-reduction principles and a patient-centered approach.
- Work collaboratively with the care team to provide compassionate, patient-centered care.
- Participate in care coordination meetings, staff meetings, and training.
- Provide client outreach in settings such as homeless shelters and on the street when necessary.
