    Description

    We are looking for a contingent applicant interested in an average of 16 hours per week. The hours will be 8:00 a.m.- 4:30 p.m. or 10:00 a.m.- 6:30 p.m.

    Qualification Requirements

    • Graduate of an accredited school of registered nursing; BSN preferred.
    • Currently licensed as an RN through the Ohio Board of Nursing.
    • Minimum 2 years of clinical experience in acute setting; homecare or hospice experience preferred.
    • Has working experience charting in an EMR system, Netsmart system preferred.
    • Possesses clinical expertise related to critical thinking, assessment, problem-solving, and technical skills.
    • Possesses the ability to provide basic and skilled nursing care, set priorities of care, and appropriately assess limits of intervention.
    • Possesses strong verbal and written communication skills; demonstrates ability to communicate effectively with patients, families, and team members regarding care issues.
    • Has the ability to work independently and within an interdisciplinary team.
    • Demonstrates strong organizational, time management, and prioritization skills.
    • Understands the hospice philosophy, principles of death/dying, concepts of pain/symptom management, and adult teaching/learning principles.
    • Must be a licensed driver with dependable automobile that is insured in accordance with state and/or organization requirements.
    • Must be CPR certified according to the American Heart Association CPR standards.

    Essential Job Responsibilities

    • Answers incoming phone calls from patients and families and communicates information to the primary RN or other team member as required.
    • Redirects nursing visits as needed.
    • Assists in responding to informational/referral phone calls.
    • Maintains communication log book for evening/night service needs.
    • Maintains accurate information on Patient/Family profile, including medication profile as required.
    • Follows up with lab results and schedules lab work as needed.
    • Assists in processing and managing physician orders.
    • Assists with home visits/admissions as needed.
    • Assists with record review.
    • Forwards referrals to massage and music therapists.
    • Participates in in-services or other continuing education to meet own needs and agency requirements.
    • Responsible to learn and utilize technology made available by the agency to increase efficiency, communication and performance of duties.
    • Attends staff meetings and completes all mandatory training.
    • Holds in strictest confidence all patient information and discloses information and data only to persons authorized by Hospice.
    • Performs other duties as assigned.
