Saint Luke's Hospital is looking to add a RN, Care Coordinator to the Care Connections team.

The ideal candidate will have previous care coordination or case management experience in an acute care setting.

BSN Degree required within 3 years of start of position.

Shift: Full time Days

Job Description:

Care Connections Nurse is part of a team designed to effectively assist patients in transitioning from hospital to community. Care Connections Nurse will work with the patient to focus on collaboration between hospital and community-based providers and arranging post discharge services. Care Connections Nurse functions as a member of a multidisciplinary team with a goal of improving the patient's health status and reducing the need for hospital care. Duties may also include assistance with program development, evaluation, professional education, or supervision of students.

Job Requirements

Applicable Experience:

1 year

Basic Life Support- American Heart Association or Red Cross, Registered Nurse- Various

Bachelor's Degree- Science in Nursing
