About the Position
An "RN Care Coordinator" coordinates patient transition along the healthcare continuum through assessments, referrals and interdisciplinary planning for the high risk patient population. The Care Coordinator focuses on the education, social needs and on-going interactions with the patient, in the acute, post-acute and outpatient setting. The Care Coordinator works closely with the patient, their support system, physicians, and outside support agencies; such as home health care (HHC) agencies, extended care facilities (ECF), home medical equipment (HME) agencies, etc. The goals of the Care Coordinator are: prevention of avoidable admissions by frequent contact with the patient through phone calls and presence during their physician appointments, education and survival skills on specific diseases, coordination of needs and resources based on individual patient needs, and improvement in the quality of life for patients and their families.
Overview of Responsibilities
Must have comprehensive knowledge of the nursing process and nursing practice and their application, including principles of safety and infection control (trends, practice and application).
Assures standards of Indiana State Department of Health, HFAP and other regulatory agencies are met.
Must possess critical thinking skills and ability to make independent decisions.
Possesses awareness of health care delivery systems and level of educational preparation of health care providers.
Demonstrates application of principles of personnel administration in assigning, supervising, coaching and evaluating.
Recognizes and reinforces positive efforts of the health care team/co-workers.
Demonstrates skills of effective communication to ensure consistent, clear and concise communications. Effective communication must be consistently utilized to assure continuity of patient care and prompt resolution of problems/concerns.
Demonstrates effective problem solving using logic and sound decision making models.
Directs and delegates care based on identified need, skill and competence of other care providers.
Demonstrates that she/he can work independently within scope of practice.
Coordinates care with other disciplines.
Uses appropriate resources - Pharmacy, Rehab Services, etc.
Compliant with attendance policy.
Collaborates with healthcare team and management to develop cost-effective solutions, improve throughput, improve patient outcomes, arrange & coordinate patient needed services.
Directs and delegates care based on identified need, skill and competence of other care providers

Education & Experience
Experience Required: At least 3 years clinical experience in an acute care setting required
Education Preferred: Bachelor's Degree preferred
Experience Preferred: Knowledge of Community Services preferred
Licensure & Certification
Licensure in the State of Indiana as a Registered Nurse is required
Must maintain competence in BLS after initial training and certifications in appropriate field require
