Opportunity for an RN to join our Women's Health Clinic team as a full-time Care Coordinator.  As the Care Coordinator you will work with the team to support patients with chronic conditions and/or complex needs. You will develop, manage, and update highly individualized care plans for individuals at-risk for poor outcomes and more!

QUALIFICATIONS:

Education:

• Current Colorado RN license or ability to obtain.
• BLS Certification through the American Heart Association

 Skills:

• Effective communication – both verbal and written
• Excellent interpersonal skills
• Ability to affect change.
• Ability to perform critical analysis.
• Self-Directed
• Positive Attitude

 Experience:

• Minimum 2 years RN work experience.
• Three to four years RN experience in the area of specialty is preferred.
• Experience in Case Management a plus.

SALARY RANGE

• $36.76 to $54.06‎/hour (BSN)
• Entry salary dependent upon education, skill set, and experience.
• Valley View recognizes a 4% pay difference for RN-ADN, BSN, MSN
