RN Care Manager

Job Title: RN Care Manager
Location: Bronx, NY
Duration: 08/03/2026 11/07/2026
Shift: 10:00 AM-6:00 PM | 7.00 Hours/Day | 35.00 Hours/Week
Pay Range - $60/hr - $65/hr.

Notes:

• Experience with Care Coordination, Utilization review and discharge planning Computer literacy.
• Five (5) years minimum of clinical experience as a Registered Nurse.
• Experience with Care Coordination, Utilization review and discharge planning Computer literacy.
• Bachelors or Masters in Nursing is required.
• RN Licensed as a Registered Nurse in the State of New York.
• PRI and Screen certification during submission.
• Primary Source Verification of License and Registration of Registered Professional Nurse from NYS Department of Education (NYSED).
• AHA BLS.
• Local candidates are strongly preferred.

Duties:

• RN Licensed as a Registered Nurse in the State of New York.
• PRI and Screen certification within three months of employment.
• Primary Source Verification of License and Registration of Registered Professional Nurse from NYS Department of Education (NYSED).
• BLS.

Preferred Certifications & Licensure:

• Case Management Certification.
