    Job Summary:
    Tryfacta is seeking a RN--Care Manager for our client in Chico CA 95926. This is a 2-year contract assignment. If you meet the qualifications listed below and are interested, please Apply Now!

    Position Title: RN--Care Manager
    Location: Chico CA 95926
    Duration: 2 years
    Work Schedule: 8-hour shifts, 8am - 4:30pm, weekends and weekdays

    Responsibilities for this position include, but are not limited to:
    Coordinate patient care and services throughout the continuum of care, from admission through discharge.
    Perform comprehensive patient assessments, care planning, implementation, and evaluation using the nursing process.
    Collaborate with physicians, patients, families, interdisciplinary teams, and payors to ensure timely, high-quality, and cost-effective care.
    Develop and coordinate safe discharge plans, post-acute services, and appropriate transitions of care.
    Conduct utilization review, evaluate admission and discharge criteria, observation status, readmissions, and resource utilization.
    Identify discharge barriers and facilitate timely interventions to optimize patient outcomes.
    Advocate for patients and families while serving as a liaison between healthcare providers and community resources.
    Coordinate clinical pathways based on patient diagnoses and support utilization management initiatives.
    Monitor patient progress throughout the episode of care and ensure continuity of services until discharge or transfer.
    Participate in quality improvement initiatives, outlier reviews, and compliance with Medicare, regulatory, and hospital standards.
    Provide care coordination support in the Emergency Department as assigned.
    Maintain accurate clinical documentation and communicate effectively with the multidisciplinary healthcare team.

    Active and unrestricted California Registered Nurse (RN) License.
    Minimum 1 year of recent Acute Care Case Management experience.
    Current BLS/CPR Certification.
    Strong experience in Care Coordination, Discharge Planning, Utilization Review, and Transitions of Care.
    Knowledge of admission/discharge criteria, observation status, Medicare guidelines, and resource utilization.
    Experience collaborating with physicians, multidisciplinary teams, patients, families, and payors.
    Proficiency with EPIC EMR and accurate clinical documentation.
    Excellent communication, critical thinking, organizational, and patient advocacy skills.
    Ability to coordinate post-acute services and manage multiple patient cases in a fast-paced acute care environment.
    Flexibility to support the Emergency Department and float as needed according to facility requirements.
