What You’ll Be Doing

  • Deliver high-quality, compassionate nursing care across clinic, home, and community settings

  • Conduct comprehensive patient assessments and develop individualized care plans

  • Coordinate care across providers, specialists, and support services

  • Manage chronic and acute conditions, ensuring seamless transitions of care

  • Perform medication reconciliation and monitor treatment adherence

  • Educate patients and families on health management and safety

  • Participate in a 24/7 on-call rotation for patient support

  • Collaborate with CNAs and care teams to ensure optimal patient outcomes


What You Bring

  • Active RN license (required)

  • 1+ year of nursing experience

  • Strong communication and care coordination skills

  • Passion for working with elderly or medically complex populations

  • Ability to work independently while thriving in a team environment

  • Reliable transportation (travel required for home visits)


Bonus Points If You Have

  • Experience in home care, managed care, or long-term care

  • Familiarity with EMR systems

  • BSN (preferred)

  • Strong understanding of patient-centered care models
