RN, Care Manager (Home Health)

Registered Nurse (RN) – Care Manager, Home Health (Contract)

Location: Oregon 97216
Pay: $2,743 – $2,905/week

Schedule

• 8-hour Day Shift (1st Shift)

• Monday – Friday

• Shift Time: 8:00 AM – 5:00 PM

• No routine call or float (may be assigned based on agency needs)

Position Summary & Responsibilities

• Manage home health caseload and coordinate patient care plans

• Perform assessments, implement care, and monitor patient progress

• Collaborate with interdisciplinary teams to meet care goals

• Educate patients and families and ensure timely, accurate documentation

Job Requirements

Education & Experience:

• BSN: Preferred

• Acute care or home health experience: Preferred

Licenses/Certifications (Required):

• Active RN license in the state of Oregon

• BLS (AHA/HS-BLS/RQI)

• Valid Driver’s License (age 21+)

• Current auto insurance and reliable vehicle
