RN Care Manager II

MavenHealth Solutions is partnered with a well-established care management group that’s been delivering highly personalized, patient-first care across the Austin area for over 20 years. Their approach is rooted in collaboration, transparency, and showing up for patients and families during some of the most complex and challenging moments of their lives.

They are currently seeking a compassionate Care Manager RN who brings a servant-leadership mindset to their work. This role is centered on care coordination for medically complex patients and their families in a field-based environment, throughout the greater Austin metro. 

No two days look the same, your office might be your car, a patient’s home, or a provider visit. It’s ideal for someone who enjoys autonomy, can confidently navigate complex situations, and values building meaningful, long-term patient relationships. If you prefer a predictable desk role, this likely isn’t it, but if you’re energized by being out in the field and advocating in real time, it could be a strong fit.

What’s being offered:

• Full-time, permanent position; Mon-Fri with full benefits and PTO starting at the 60 day mark 
• Salary range: $80,000–$92,000/year (based on experience)
• Mileage reimbursement + monthly tech stipend
