We are seeking a compassionate and highly independent Registered Nurse (RN) Care Manager to support perinatal and postpartum patients within an OB/GYN practice. This hybrid role combines virtual care with in‑clinic presence and requires strong clinical judgment, a high level of autonomy, and the ability to effectively prioritize patients for proactive outreach and follow‑up.

Pregnancy and the postpartum period are among the most medically vulnerable times in a patient’s life. As an RN Care Manager embedded within an OB/GYN practice, you serve as a critical clinical partner across the full perinatal continuum — from prenatal risk identification through postpartum recovery.

Your expertise supports early detection of emerging complications, strengthens care transitions, and ensures high-risk patients do not fall through the cracks. By bridging obstetric, primary care, pediatric, and behavioral health services, you help reduce preventable complications, improve maternal and infant outcomes, and advance health equity.

• Provide autonomous care coordination, education, and follow‑up for prenatal and postpartum patients.
• Prioritize patient outreach based on clinical risk, alerts, patient‑reported symptoms, and provider requests.
• Conduct virtual and in‑person outreach to support care plans, symptom monitoring, and appointment adherence.
• Identify and escalate clinical or psychosocial concerns using strong independent assessment skills.
• Collaborate with a multi-disciplinary team including OB/GYN providers, primary care, pediatricians, behavioral health, nurse practitioners, and community resources.
• Document all patient interactions within the Electronic Medical Record (EMR).
• Use Microsoft Outlook, Teams, and Office Suite for communication and care workflows

• Unrestricted Maryland RN license or compact state license; BSN preferred
• 2–4 years of nursing experience, preferably in OB/GYN, labor and delivery, postpartum, NICU, or newborn nursery.
• 1–2 years of care management experience, preferably working with perinatal or postpartum populations.
• Demonstrated ability to work with a high degree of autonomy in both virtual and clinic settings.
• Strong assessment, prioritization, and critical thinking skills.
• Proficiency with EMR systems and virtual care technologies.
• Comfort working independently and as part of a multidisciplinary team
• Knowledge of Medicaid and commercial payor plans a plus
