RN - Care Navigator (full-time)

NeoPath Health, Inc., a Minnesota based administrator of employer based medical services, is in search of a full-time RN - Care Navigator to work at an employer-sponsored primary care clinic in the Green Bay area.

Position Summary

The Care Navigator is responsible for assisting patients to obtain the right care at the right time. In collaboration with the clinical team, the care navigator will assess care needs, assist with the referral process, provide outreach to ensure that care gaps are closed, and serve as a patient advocate. This position requires a high degree of interpersonal and organizational skills. Principal Duties and Responsibilities

• Demonstrate personal responsibility and respect for patients, families, caregivers, and colleagues by displaying professionalism, critical thinking skills, and customer service with every interaction.
• Assist the care team(s) by managing the referral process, following up with care partners, and communicating with the patient along the way.
• Research, find, and link patients to resources, services, specialists, and preferred partners to support care needs.
• Conduct and document assessments to identify patient needs and any resources for effective self-care management.
• Act as a primary contact point, advocate, and source of information for patients, and the community partners who treat them.
• Document each patient's individualized plan of care and care coordination efforts in their patient record.
• Provide timely communication with patients and care partners as required.
• Develop and maintain relationships with patients, families, caregivers, and the patient's care team.
• Assist with clinic triage efforts, to include clinical guidance and support, while connecting patients to appropriate care.
• Attend meetings to increase knowledge of current health issues and trends in the healthcare marketplace.
• Identify gaps in care and proactively work to improve access and communication across the care continuum.
• Share pertinent plan information with clinical team members to support referral process.
• Other duties as assigned.

Qualifications

• Graduate of an accredited nursing program.
• Current RN and BLS certification required.
• Basic computer skills required, EHR experience preferred.
• Exemplary work ethic and professionalism.
• Demonstrated skills in leadership, advocacy, communication, education, and patient counseling.
• Excellent interpersonal skills - reflecting clarity and diplomacy.

Full-time 36-40 hours per week. No weekends, holidays, or after-hours call.
