Job Title: RN: Case Manager

Location: Jeffersonville, IN 47130

· Work Area/Coverage: Harrison, Clark, Crawford, Orange, partial Floyd & Washington counties (plus additional areas as needed for PTO/call-ins/admissions)

Pay: $56.14/hour

· Mileage Reimbursement: $0.46/mile

Schedule: Monday-Friday, 8:00am-5:00pm + weekend on-call rotation

Overview

We’re hiring an RN Case Manager to complete comprehensive assessments for high-risk members and coordinate care across providers and settings. This role focuses on care planning, care transitions, patient advocacy, and driving quality outcomes through effective clinical case management.

Key Responsibilities

• Perform comprehensive assessments of high-risk members
• Collaborate with primary care providers to implement an individualized care plan
• Identify, address, and continuously reassess appropriate levels of care
• Coordinate care transitions with physicians, social workers, discharge planners, and other stakeholders
• Put non-medical supports in place to support treatment plan compliance (e.g., transportation/housing resources as applicable)
• Maintain detailed documentation of clinical, functional, and fiscal outcomes

Required Skills & Qualifications

• Active RN license in Indiana (compact license accepted)
• Home Health experience (2+ years recent)
• HCHB (Homecare Homebase) & OASIS experience 
• Strong clinical skills including: Wound care, IV therapy, Drain care, G-tubes, Patient education
