Registered Nurse Case Manager - Beyond Hospice

For more than 130 years, Episcopal Homes of Minnesota has been a trusted leader in senior care in St. Paul's Midway neighborhood. Beyond Hospice, a service of Episcopal Homes, is seeking an experienced and compassionate Registered Nurse Case Manager to provide meaningful end-of-life care and support to patients and their families.

This is a great opportunity for an RN who enjoys independence, relationship-centered care, and being part of a supportive interdisciplinary team.

Pay & Schedule

Pay: $37.44-$56.66/hour, based on experience

Schedule: Monday-Friday, 8:00 a.m.-4:30 p.m.

Weekend Rotation: Every 6th weekend

What You'll Do

• Manage and coordinate care for an assigned group of hospice patients

• Complete nursing assessments and individualized plans of care

• Provide skilled nursing care focused on comfort, symptom management, and quality of life

• Educate and support patients, families, and caregivers throughout the hospice journey

• Coordinate medications, medical equipment, supplies, and community resources

• Collaborate with physicians, social workers, chaplains, therapists, and other members of the hospice team

• Communicate changes in condition and obtain provider orders as needed

• Maintain timely and accurate clinical documentation

• Participate in interdisciplinary team meetings and the on-call rotation

Qualifications

Required

• Current Minnesota Registered Nurse (RN) license

• Graduate of an accredited school of nursing

• Strong assessment, communication, critical-thinking, and documentation skills

• Valid driver's license, reliable transportation, and current auto insurance

Preferred

• Bachelor's degree in Nursing (BSN)

• Two years of acute care nursing experience

• Two years of recent hospice or home health experience
