Position Summary

The RN Case Manager & Rural Outreach Coordinator is a Registered Nurse who provides, direct nursing services, and nursing leadership direction for services delivered and outreach to support and optimize continuity of care in rural Nevada. The role is in keeping with applicable Nevada Revised Statutes (NRS) and the Nevada Administrative Code (NAC). This position is also responsible for actively participating in the training for clinical support staff across the organization to optimize continuity of care and improve clinical outcomes.

Essential Duties and Responsibilities

  • Works in partnership with the Practice Manager to assist with the team-based care model.
  • In partnership with the Practice Manager, is responsible for overseeing the Home Visit Team, the schedule, travel, and activities to improve patient access.
  • Responsible for monitoring utilization and reporting of Remote Patient Monitoring Programs
  • Assists with the assessment of medical support staff competency and ability to identify training opportunities.
  • Works closely with Population Health and Elko County Health Department to identify the targeted high-risk population within practice site(s) per PCP referral, risk stratification, and patient lists. Includes patients with repeated social and/or health crises.
  • Responsible for working collaboratively with all internal and external healthcare team members.
  • Responsible for coordinating and participating in any (in scope of license) necessary in-office services or home visits to support patient access to care.
  • Triages walk-ins and phone calls at the health centers, where applicable.
  • Participates as a leader in the administration of NVHC programs, such as the immunization program, to ensure to ensure compliance and a best practice model.
  • Administers medications and immunizations to pediatric and adult patients at NVHC clinics and health fairs.
  • Responsible that the monthly laboratory quality assurance checks are conducted at all medical and WIC clinics by the RN or delegate.
  • Participates in direct patient care and co-visits with MD as applicable. RN is a key member of the team that provides services under the Community Chest contract with Storey County (Carson City area RN only).
  • Acts as a liaison for employee health-related matters.

Required Qualifications

  • Graduate of an accredited nursing school
  • Licensure as a registered nurse in the state of Nevada
  • Current CPR or BLS certification
  • Minimum three years of experience in a clinical practice with good clinical skills/ability to function as a staff nurse
  • Active Nevada Driver's License

Preferred Qualifications

  • Bachelor's degree in nursing
  • Knowledge of the essential functions, practices and procedures of a medical clinic and office
  • Knowledge of in-office lab procedures; pediatric and adult phlebotomy skills
  • Knowledge and skill in pediatric and adult medication and immunization administration
  • Ability to interact effectively and positively with other staff members
  • Detail oriented and ability to handle multiple and shifting priorities
  • Excellent ability to problem solve, deescalate/resolve conflict and perform service recovery.
  • Demonstrated ability to produce high quality work in a consistent manner
  • Demonstrated ability to manage timelines and projects successfully
