The purpose of the RN case manager to serve as case manager for Medicaid patients and help maintain stabilization, delay/prevent deterioration of health status, manage chronic conditions, and/or improve health status for patients while performing OASIS start of care, transfer, resumption, and discharge.
QUALIFICATIONS:
License, required
2+ year nursing experience, required
1+ year Home Health nursing experience, required
Case manager experience, preferred
Must demonstrate excellent interpersonal skills; both written and verbal, time management, and Microsoft Office skills
ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY
Completes start of care OASIS for patients to begin home health services as well as regular re-certifications.
Initiates the plan of care which establishes patient-centered goals as approved by physicians.
Responsible for the completion of all home health documentation including admissions, re certifications, discharges, and all related OASIS documentation.
Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested.
