Position Summary: Responsible for providing comprehensive case management services to individuals seeking substance use treatment or have been referred for medication-assisted treatment. Responsible for ensuring that all critical tasks are fulfilled on a timely basis.

Critical Tasks:

  • Provide active case management for clients identified as appropriate for the addiction medicine team..

  • Performs primarily as a Registered Nurse, advocating for patient welfare, and serves as a liaison between patients, their families, and various healthcare providers.

  • Electronically documents all patient information and observations on a daily basis within no more than 48 hours (2 business working days), preferably before.

  • Provides outreach, support, and resource coordination to individuals who have experienced an overdose to promote engagement in treatment and reduce risk for recurrence.

  • Attends scheduled medication-assisted treatment team meetings.

  • Provides education for Clinical Liaisons, Medical Assistants, Community Members and other groups as identified.

  • Interviews potential clients in crisis situations to develop background and details of problems. Assesses and evaluates cases and prepares and implements treatment and case management plans.

  • Works closely with families of client and provides appropriate guidance to family members.

  • Maintains working relationships with staff of other agencies and institutions, homes, and facilities, acting as liaison between clients, agency, or institution.

  • Works collaboratively with medical staff serving clients. Provides consultation to staff on specific cases involving clients.

  • Prepares and maintains written case records, reports, and forms, performs case follow-up and closing, and performs other administrative tasks as required.

  • Coordinates with other Gila River Health Care departments regarding referrals, screenings, and/or treatments.

  • Coordinates data with Gila River Health Care's Outcomes Management and Informatics systems.

  • Collects, analyzes and responds to data from external sources (e.g., Gila River Indian Community Department of Rehabilitation and Supervision, Police Department, GRIC Crisis Response Team, etc.).

  • Assists in and assures proper distribution/usage of Naloxone kits into the homes of individuals at risk for opioid overdose.

  • Demonstrates ability to regularly change priorities to accomplish all tasks despite frequent interruptions.

  • Collaborates with other Case Managers regarding their caseloads as needed and appropriate.

  • Demonstrates ability to clearly communicate, both orally and in writing, while performing all essential functions.

  • Maintains exclusive focus upon patients, the quality of their care, and their individual needs without significant distraction by administrative and non-patient contact activities.

  • Performs other special projects and duties as assigned.

Required Qualifications:

  • Current and valid State of Arizona Registered Nurse license; BSN preferred.

  • Must have at least four (4) years of nursing experience including at least one (1) year full time patient care experience in an acute care, home health, long term care, or related healthcare setting. Experience with substance abuse patients strongly preferred.

  • Must have a CPR certificate or obtain within 30 days of hire.

  • Current and valid fingerprint card.

  • Demonstrated ability to relate to diverse cultures and specifically the Gila River Community and/or other Native American cultures.

  • Interest in medical subjects, patient services/care, and customer service.

  • Prior work experience within a medical environment is preferred.
