Pay Range: Regular Pay: $42/hr
Weekly Housing & Meal Stipend: $1680/weekly
Weekly Per Diem: $1021.72
Weekly Gross Pay: $ 2,701.72(subject to completion of scheduled hours)
Key Responsibilities:- Coordinate and manage patient care from admission through discharge or transfer.
- Assess, plan, implement, and evaluate patient care using the nursing process.
- Collaborate with physicians, nurses, therapists, patients, families, and insurance providers to develop and coordinate individualized care plans.
- Review admissions, discharge planning, observation stays, readmissions, and utilization management to ensure appropriate level of care.
- Initiate and monitor clinical pathways based on admission diagnoses.
- Advocate for patients and families while facilitating communication among the healthcare team.
- Evaluate quality and utilization cases to improve patient outcomes and resource management.
- Maintain accurate documentation using EPIC EMR.
- Support patient transitions to appropriate levels of care and community resources.
- May provide case management support in the Emergency Department as assigned.
- Contract Length: 13 Weeks
- Shift: 8-hour shifts, 8am - 4:30pm, weekends and weekdays
- Scheduled Hours: 40 hours [Hours per Facility]
Key Requirements and Technology Experience:- Active California Registered Nurse (RN) License.
- Minimum of 1 year of recent Case Management experience in an acute care setting.
- Current BLS/CPR Certification.
- Strong knowledge of discharge planning, utilization review, and care coordination.
- Experience with EPIC EMR preferred.
