  The Role You'll Play<\/span><\/b>
  <\/p>

  As the primary nurse overseeing your patients, you'll become the clinical leader coordinating every aspect of their care.<\/span>
  <\/p>

  You'll partner with physicians, therapists, caregivers, and family members to create individualized care plans while helping patients avoid unnecessary hospitalizations and maintain their independence.<\/span>

  What You'll Be Doing<\/span><\/b>
  <\/p>

  • Manage an assigned <\/span>home health<\/span><\/b> caseload throughout the <\/span>St. Paul<\/span><\/b> territory<\/span>
    <\/li>
  • Complete <\/span>OASIS<\/span><\/b> Start of Care, Recertification, and Discharge assessments<\/span>
    <\/li>
  • Develop and update individualized care plans<\/span>
    <\/li>
  • Perform skilled nursing visits and comprehensive patient assessments<\/span>
    <\/li>
  • Educate patients and caregivers on medications, diagnoses, and treatment plans<\/span>
    <\/li>
  • Coordinate care with physicians and interdisciplinary healthcare teams<\/span>
    <\/li>
  • Complete documentation using <\/span>Homecare Homebase (HCHB)<\/span><\/b>
    <\/li>
  • Monitor patient progress and adjust care plans as clinical needs change<\/span>
    <\/li><\/ul>

    Who We're Looking For<\/span><\/b>
    <\/p>

    • Active Minnesota Registered Nurse (RN) license<\/span><\/b>
      <\/li>
    • Three or more years<\/span><\/b> of RN experience<\/span>
      <\/li>
    • Home health experience preferred, but experienced clinicians from other settings are encouraged to apply<\/span>
      <\/li>
    • Strong assessment, organization, and critical thinking skills<\/span>
      <\/li>
    • Comfortable working independently while managing a daily schedule<\/span>
      <\/li>
    • Reliable transportation for travel throughout the <\/span>St. Paul<\/span><\/b> service area<\/span>
      <\/li><\/ul>
