Contract Duration: 13 Weeks
Shift: Day Shift | 5x8s (8:00 AM – 4:30 PM)
Position Overview
"Navitas Healthcare, LLC" is seeking an experienced Registered Nurse (RN) Case Manager for a 13-week contract assignment in Santa Rosa, California. The role is based in an acute care hospital setting with a strong focus on discharge planning, care coordination, and utilization support.
Key Responsibilities
- Coordinate patient care plans and discharge planning
- Collaborate with interdisciplinary teams to ensure continuity of care
- Manage utilization review and appropriate level-of-care placement
- Document all case management activities in EMR system
- Support timely transitions from acute care to post-acute settings
Requirements
- Active California Registered Nurse (RN) License
- Minimum 2 years of Case Management experience in an acute care setting
- Strong background in discharge planning and care coordination
- Experience with Epic EMR preferred
- Strong communication and interdisciplinary collaboration skills
