Job Summary:
We are seeking an experienced and compassionate Registered Nurse (RN) – Case Manager to coordinate patient care, facilitate effective care transitions, and ensure the delivery of high-quality, cost-effective healthcare services. The ideal candidate will collaborate with interdisciplinary teams, assess patient needs, develop individualized care plans, and support optimal patient outcomes while ensuring compliance with clinical standards and organizational policies.

Job Description:

• Assess patients’ medical, psychosocial, and care coordination needs.
• Develop, implement, and monitor individualized care plans.
• Coordinate patient care with physicians, nurses, and interdisciplinary teams.
• Facilitate discharge planning and ensure smooth transitions of care.
• Conduct ongoing reviews of patient progress and treatment plans.
• Monitor resource utilization and promote cost-effective care delivery.
• Identify barriers to care and coordinate appropriate interventions.
• Collaborate with healthcare providers, patients, and families to establish care goals.
• Maintain accurate clinical documentation and case management records.
• Support continuity of care and ensure adherence to established clinical guidelines.
• Assist with referrals, follow-up appointments, and community resource coordination.
• Advocate for patients to ensure timely access to appropriate healthcare services.

 
Requirements:

• Active California RN License
• 1+ year of experience in Case Manager, Acute Care, Discharge Planning
• BLS
