Job Title: Registered Nurse – Case Manager
Location: Eureka, CA

Pay Rate:
Local: Up to $2400 Weekly
Traveler: Up to $2800 Weekly (Stipend Included)
Duration: 13 Weeks
Shift & Schedule: Day Shift | 5x8 Hours (08:00 AM – 04:30 PM) | 40 Hours Weekly | Every Other Weekend Required
Position Overview:
Navitas Healthcare, LLC is seeking Registered Nurse – Case Manager for an exciting Travel or Local job in Eureka, CA.
Primary Responsibilities:
• Coordinate and manage patient care plans across the continuum of care
• Perform discharge planning and utilization management
• Collaborate with physicians, nursing staff, and interdisciplinary teams
• Assess patient needs and facilitate appropriate resource allocation
• Maintain accurate documentation within Epic EMR system
• Support patient advocacy and continuity of care initiatives
• Float between like units based on facility needs
Requirements:
• Active RN License required
• Minimum 2 years of Case Management experience required
• Acute care hospital experience required
• Experience with Epic charting preferred
• Strong communication and care coordination skills
• First-time travelers accepted with clinical approval
• Associate Degree or BSN from an accredited institution required
• BLS Certification required
Additional Information:
• No on-call requirement
• Locals accepted
• Travelers may float between like units as needed
