Position Overview
We are seeking an experienced RN Case Manager to provide case management, care coordination, discharge planning, and transition-of-care support across assigned campuses.

Required Qualifications

• Minimum 2 years of Case Management experience.

• Current RN license required and verifiable through Nursys.

• BLS certification required.

• Degree accreditation must be listed on the candidate profile.

• Willingness to float to sister campuses as needed.

• Travelers may be required to float to other sister campuses.

• Must meet the 50-mile radius requirement from the facility to permanent address.

• Strong communication and teamwork skills with the ability to function as part of the core team.

• Firm start date required.

• Care Management certification such as ACM or CCM preferred.

Responsibilities

• Provide professional RN case management and care coordination.

• Assess patient needs and coordinate appropriate services throughout the continuum of care.

• Collaborate with physicians, nursing staff, social workers, and other members of the healthcare team.

• Support appropriate discharge planning and transitions of care.

• Coordinate resources and services based on patient needs.

• Float to sister campuses as required.

• Function as part of the core team and follow facility workflows, policies, and expectations.
