Baywood Home Care is looking for RN Case Managers to deliver personalized care across the Twin Cities metro area, with a company vehicle and benefits provided.

What You'll Do: 

• Evaluate each client’s condition and create personalized care strategies to promote overall well-being
• Collaborate with healthcare providers and support staff to deliver and oversee care services
• Continuously revise care approaches based on changes in client status and response to treatment
• Guide clients and families on medical conditions, medications, and effective care routines
• Document all interactions thoroughly while adhering to clinical standards and home care requirements

What You Will Need: 

• Active Registered Nurse (RN) license in the state of MN required
• Home care experience required
• Must be 21 years of age
