Job Description:

We are seeking a dedicated RN Case Manager for a 26-week contract position at a group home in Bronx, NY. The ideal candidate will provide compassionate care and support to residents while ensuring their safety and well-being. The role involves coordinating with healthcare providers, developing care plans, and advocating for residents' needs.

Responsibilities:

• Assess residents' health and psychosocial needs
• Develop and implement individualized care plans
• Coordinate healthcare services and community resources
• Monitor residents' progress and adjust care plans as needed
• Advocate for residents' rights and preferences
• Provide education and support to residents and their families
• Maintain accurate and up-to-date documentation

Requirements:

• Current RN license in New York
• PRI Certification
• Minimum of 1 year of experience in case management, preferably in a group home setting
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Compassionate and empathetic attitude
• Knowledge of community resources and healthcare systems
