Facility Location: Bronx, NY 10452 Shift: Day, Monday-Friday, 8:00 AM - 4:30 PM Rate: $52/hr Contract Length: 26 weeks
Job Description:
We are seeking a dedicated RN Case Manager for a 26-week contract position at a group home in Bronx, NY. The ideal candidate will provide compassionate care and support to residents while ensuring their safety and well-being. The role involves coordinating with healthcare providers, developing care plans, and advocating for residents' needs.
Responsibilities:
Assess residents' health and psychosocial needs
Develop and implement individualized care plans
Coordinate healthcare services and community resources
Monitor residents' progress and adjust care plans as needed
Advocate for residents' rights and preferences
Provide education and support to residents and their families
Maintain accurate and up-to-date documentation
Requirements:
Current RN license in New York
PRI Certification
Minimum of 1 year of experience in case management, preferably in a group home setting
Strong communication and interpersonal skills
Ability to work independently and as part of a team
Compassionate and empathetic attitude
Knowledge of community resources and healthcare systems
