RN Case Manager

Works to enhance the quality of patient management and satisfaction and promotes continuity of care and cost effectiveness through integration of the functions of case management, utilization review/management and discharge planning.Monitors the implementation of the plan of care as determined by the physician and works collaboratively with the multidisciplinary team to facilitate achievement of desired treatment outcomes.
