RN Case Manager - Home Health Agency

Responsibilities:

• Conduct comprehensive patient assessments to develop personalized care plans.
• Coordinate and manage patient care activities across interdisciplinary teams.
• Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
• Provide education and support to patients and their families regarding health management.
• Document all patient interactions and care plans in compliance with agency policies.
• Collaborate with physicians and other healthcare professionals to enhance patient care.
• Participate in quality improvement initiatives to enhance service delivery.
• Ensure adherence to regulatory requirements and best practices in home health care.

Requirements:

• Current RN license in the state of Indiana.
• Minimum of 2 years of nursing experience, preferably in home health or case management.
• Strong assessment, organizational, and communication skills.
• Ability to work independently and manage a diverse caseload.
• Experience with electronic health records (EHR) systems.
• Compassionate demeanor with a focus on patient-centered care.
• Basic Life Support (BLS) certification required.
• Knowledge of Medicare and Medicaid regulations is a plus.
