RN Case Manager Responsibilities


As a Home Health Registered Nurse (RN), you will play a vital role in coordinating and delivering high-quality patient care in the comfort of clients’ homes.


Responsibilities include:


  •  Conduct initial nursing assessments and ongoing patient evaluations 
  •  Develop and implement individualized care plans 
  •  Provide skilled nursing care according to physician orders and care protocols 
  •  Maintain accurate clinical documentation and progress notes 
  •  Supervise, educate, and support caregivers and nursing staff 
  •  Ensure compliance with HIPAA, state regulations, and federal healthcare standards 
  •  Collaborate with physicians, families, and interdisciplinary care teams 
  •  Participate in quality improvement initiatives and staff training programs 
  •  Monitor patient outcomes and recommend adjustments to care plans as needed 
  •  Promote a positive patient and family experience through compassionate care 
Qualifications:


  •  Current unrestricted Registered Nurse (RN) license 
  •  Graduate of an accredited nursing program 
  •  Bachelor of Science in Nursing (BSN) preferred 
  •  3–5 years of clinical nursing experience preferred 
  •  Previous home health, hospice, case management, or primary care experience preferred 
  •  Strong nursing assessment and clinical decision-making skills 
  •  Excellent communication and leadership abilities 
  •  Ability to work independently and manage multiple patients effectively 
  •  Reliable transportation, valid driver’s license, and auto insurance required 
  •  Ability to travel locally throughout the service area 
  •  Knowledge of state and federal home health regulations preferred
