Connected Health Care is partnering with a leading healthcare organization to recruit an experienced Home Health Case Manager RN for a full-time opportunity in Meridian, ID. The Home Health RN Case Manager will play a key role in assessing patients, developing individualized plans of care, coordinating clinical services, completing documentation, and helping patients achieve their healthcare goals in the comfort of their own homes.
Job Description
Provide skilled nursing care to patients in their homes while promoting safety, independence, and quality of life.
Serve as the primary RN Case Manager and coordinate the overall plan of care for an assigned patient caseload.
Perform comprehensive nursing assessments and evaluate patients' physical, psychological, and social needs.
Develop, implement, and regularly update individualized plans of care based on patient needs and physician orders.
Coordinate care with physicians, therapists, social workers, aides, caregivers, and other members of the interdisciplinary healthcare team.
Complete accurate and timely clinical documentation in accordance with agency, state, and federal requirements.
Perform and document OASIS assessments as required for home health patients.
Monitor patient progress and identify changes in condition requiring intervention or physician notification.
Educate patients and family members regarding medications, disease processes, treatment plans, symptom management, and appropriate home care.
Review medications and help identify potential medication-related concerns or discrepancies.
Communicate effectively with patients, families, physicians, and other healthcare professionals.
Participate in case conferences and care coordination activities as needed.
Promote continuity of care throughout the patient's home health episode.
Maintain compliance with applicable home health, Medicare, state, and organizational standards.
Manage time effectively while traveling between patient homes and completing required clinical documentation.
Education and Certification Requirements
Current and unrestricted Registered Nurse RN license in Idaho or eligibility for Idaho licensure.
Minimum 1 year of Home Health RN experience required.
Current BLS/CPR certification for healthcare providers.
Associate Degree in Nursing ADN or Bachelor of Science in Nursing BSN from an accredited nursing program.
Home health, Medicare, OASIS, and care coordination experience strongly valued.
Valid driver's license and reliable transportation required for traveling to patient homes.
Ability to maintain appropriate automobile insurance as required by applicable law.
Strong clinical assessment, communication, organization, and critical-thinking skills.
Preferred Qualifications
Previous experience as a Home Health RN Case Manager.
Experience completing OASIS assessments and documentation.
Familiarity with Medicare home health regulations and requirements.
Experience coordinating interdisciplinary patient care.
Familiarity with home health electronic medical records and documentation systems.
Ability to work independently while remaining connected with the clinical team.
