What Youll Do:

• Conduct thorough patient assessments and develop individualized care plans.
• Coordinate with interdisciplinary teams to ensure seamless care.
• Provide end-of-life care and symptom management for hospice patients.
• Educate patients and families on managing health conditions at home or during hospice.
• Maintain accurate and timely documentation for both home health and hospice services.

Position Details:

What Youll Bring:

• Active RN license in Idaho (or compact state)
• At least 1 year of experience in home health and/or hospice
• Excellent communication and organizational skills
• Commitment to patient-centered care
• Residency in Lewiston or within 30 minutes of Lewiston
